Date revealed for inquest into cafe siege deaths

A coronial inquest into the siege at Lindt Cafe in Sydney’s Martin Place will begin on 29 January 2015. A New South Wales Coroner’s Court official said on 14 January that the inquest will examine whether the three deaths could have been prevented, rather than apportioning blame. Jeremy Gormly SC has been selected as counsel assisting the coroner. It has been reported that a police bullet killed one of the hostages, Katrina Dawson

Adam Casselden, junior counsel assisting an inquest in the New South Wales (NSW) Coroner’s Court into a January 2013 bushfire, has examined NSW Parks & Wildlife Service senior fire ranger Peter Brookhouse. The witness stated that despite his recommendation against a backburning operation on the day when contacted by Rural Fire Service zone manager Stuart Green, his superiors went ahead anyway and ignored the windy weather conditions

Killer nurse’s CV not checked: inquest

New South Wales Deputy Coroner Hugh Dillon has begun the inquest into the deaths of 11 nursing home patients in late 2011, killed in a fire set by nurse Roger Dean. The Coroner’s Court has been told that the drug addict supplied just two references, one by his lover at the time, and that managers at the Quakers Hill Nursing Home checked neither. He was then put in charge of the night shift and had ready access to opioid painkillers. No reports were made to any disciplinary or registered body either about previous misconduct by Dean at St John of God Hospital and St George Hospital
